I've played the file through other software, both to the SD-80 (Aria Maestosa) and software GM emulation (Quicktime Player 7, which has its own set of issues but not this one). Scrubbing through the notes in the MIDI editor does not exhibit the problem! Slowing down the tempo does not correct the problem. Note that the problem only occurs in realtime playback.

You can solo channel 9 and start playback just before measure 23.4 and observe the problem. there are more note-on commands than corresponding note-off commands. There is a group of notes at measures 23.4 and 24.3 that both have stuck notes - i.e. There are two tracks in particular that are getting stuck notes, MIDI channels 4 and 9, but the notes on 9 more reliably produce the problem.
